Who We Are

At Home Harmony ACO is an Accountable Care Organization built specifically for home-based primary care. Our clinicians visit high-risk, complex older adults where they live — the patients most likely to be missed by traditional clinic-based care.
The ACO is independently governed, with a physician-led governing body accountable for quality, compliance, and the experience of every Medicare beneficiary we serve.
We align with the CMS LEAD Model and the CMS GUIDE dementia care model. That alignment lets us invest in what home-based patients actually need: longer visits, proactive coordination, 24/7 access, caregiver education, and warm handoffs to hospice and palliative care when the time is right.

At Home Harmony ACO participates in the Long-term Enhanced ACO Design (LEAD) Model, an accountable care model from the CMS Innovation Center at the Centers for Medicare & Medicaid Services (CMS). The LEAD Model supports long-term investment in primary care and rewards ACOs that improve quality and outcomes while lowering the growth in Medicare spending — with patients always coming first.
